How do I schedule an appointment?
Pick one of the avenues below and you are ready to fulfill your destiny...

Once you've scheduled your appointment:
* Download your new client forms at the New Patient tab and email completed documents to me (first save completed documents to your computer and then add as an attachment to an email to me), or bring with you to your first appointment.

Payment
Payments can easily be made at the time of your booking via PayPal. If you do not have a PayPal account.... No worries... I will send you an invoice that you can pay with your credit card.
Cash or Check at the time of your appointment is dandy too!
Do you sell supplements?
At this time UrbanNourish is in the process of manufacturing a private label line of Whole Food, Organic supplements and body care products. We hope to have this Fantastic line of products available ASAP, but until then Yvette will recommend the specific type and brand of supplements you need for your particular journey to health and wealth!!
